Clinical Observation of Cerebrovascular Accidents

Abstract
Two hundred and fourteen cases of patients with cerebrovascular accident who had been admitted to Maryknoll Hospital in Busan, Korea from October 1973 to December 1975 were studied.
1. The percentage of cerebrovascular accident patients from among the total of medical patients who had been admitted during the same period was 7.3%, and the ratio of male to female was 1.3 : 1.
2. Among the 214 cases of patients with cerebrovascular accident, there were 88 cases (41.1%) of
cerebral hemorrhage, 42 cases (19.6%) of subarachnoid hemorrhage, 74 cases (34.6%) of cerebral thrombosis, and 10 cases (4.7%) of cerebral embolism.
3. Sixty-one cases (28.5%) had the attacks in winter, 59 cases (27.6%) in fall, 52 cases (24.3%) in summer, and 42 cases (19.6%) in spring. Cerebrovascular accidents were most common in the sixth decade, and secondly in the fifth decade.
4. Hypertension was seen in 68.2% of patients with cerebral hemorrhage, 43.2% with cerebral thrombosis, and 23.8% with subarachnoid hemorrhage. Valvular heart disease was seen in 5 cases (50.0%) of cerebral embolism, and another 2 cases showed atrial fibrillation.
5. There was headache in 39 cases (92.9%) of subarachnoid hemorrhage, 49 cases (55.7%) of cerebral hemorrhage, 27 cases (36.5%) of cerebral thrombosis, and one case (10.0%) of cerebral embolism. There was vomiting in 20 cases (47.6%) of subarachnoid hemorrhage, 15 cases (17.0%) of cerebral hemorrhage, and 7 cases (9.5%) of cerebralthrombosis. Hemiplegia was found in 34 cases (38.8%) of cerebral hemorrhage, 27 cases (36.5%) of cerebral thrombosis, 7 cases (70.0%) of cerebralembolism, and 3 cases. (7.1%) of subarachnoid.hemorrhage. Coma was seen in 46 cases (52.3%) of cerebral hemorrhage, 10 cases (23. 8%) of subarachd noid hemorrhage, 8 cases (10.8%) of cerebral thrombosis, and 2 cases (20.0%) of cerebral embolism.
6. Fasting blood sugar levels over 160 mg% were seen in 26.5% of cerebral hemorrhage, 8.4% of cerebral thrombosis, and 5.0% subarachnoid hemorrhage. Serum cholesterol levels over 240mg% were seen in 25.7% of cerebral hemorrhage, 29.4% of subarachnoid hemorrhage, and 26.1% of cerebralthrombosis.
7. Cerebral hemorrhage and subarachnoid hemorrhage developed during physical activity in 26.1% and 19.0% of cases respectively Cerebral thrombosis and cerebral embolism developed during sleeping in 36.5% and 30.0% of cases respectively.
8. In thirteen patients with cerebrovascular accident, carotid angiography was performed: Among 4cases of cerebral hemorrhage, two cases of hematoma were seen, and of these 2 cases, one had arterial aneurysm in addition. Arterial aneurysm was found i 2 cases among 5 cases of subarachnoid hemorrhage. Among, 4 cases of cerebral thrombosis, one case of anterior cerebral artery occlusion was found and the other 3 had middle cerebral artery occlusion.
9. Mortality rate during hospitalization was 28.4 %, 10.0%, 4.7% and 1.4% in cerebral hemorrhage, cerebral embolism, subarachnoid hemorrhage and cerebral thrombosis respectively. Among the total patients who died, 52.0% died within 24 hours after onset. Moderate to good improvement was seen in 20.5% of cerebral hemorrhage, 52.4% of subarachnoid hemorrhage, 54. 1% of cerebral thrombosis, but no improvement was seen in cerebral embolism.
10. The recurrence rate was 8.0% in cerebral hemorrhage, 14.3% in subarachnoid hemorrhage, 9.5% in cerebral thrombosis and 20.0% in cerebral embolism.
